·540.中国中西医结合影像学杂志2023年9月第21卷第5期经直肠实时剪切波弹性成像对前列腺癌的诊断效能及其与PSA、Gleason评分的相关性王琳，宣之东，钱星宇河北省沧州市中心医院超声三科，河北沧州061000[摘要】目的：评价经直肠实时剪切波弹性成像（SWE)结合弹性模量值在前列腺癌诊断中的价值及其与Gleason评分的相关性。方法：回顾性分析经病理证实的62例前列腺恶性病变（恶性组)和59例良性病变（良性组)的弹性模量值[最大弹性模量值（Emax）、平均弹性模量值（Eman）、最小弹性模量值（Emin）],绘制ROC曲线分析SWE结合弹性模量对前列腺癌的诊断效能，并分析恶性组Emx、Eman和Emin水平与血清前列腺特异性抗原(PSA）、Gleason评分的相关性。结果：恶性组的弹性模量值（Emx、Emeamn和Emm）均明显高于良性组（均P<0.05）。恶性组EmxEman和Emimn的最佳截断值为61.71、40.89和19.25kPa时，敏感度分别为85.48%、80.65%和66.13%，特异度分别为88.14%、81.36%和62.71%。恶性组EmxEmeamEmm与血清PSA水平呈正相关（r=0.781,0.632,0.517；均P<0.01),Emx、Emam与Gleason评分呈正相关(r=0.695,0.587；均P<0.01）。结论：经直肠实时SWE结合弹性模量值可用于鉴别前列腺病变的良恶性。弹性模量值越高，表明发生恶性病变的风险越高。【关键词］超声检查，经直肠；实时剪切波；弹性模量；前列腺癌；Gleason评分Diagnosticefficacyoftransrectalreal-timeshearwaveelastographyforprostatecanceranditscorrelationwithPSAandGleasonscoreWANGLin,XUANZhidong,QIANXingyu.DepartmentoftheThirdUltrasound,CangzhouCentralHospital,Cangzhou061000,China[Abstract]Objective:Toevaluatethevalueoftransrectalreal-timeshearwaveelastography(SWE)combinedwithelasticmodulusvalueinthediagnosisofprostatecanceranditscorrelationwithprostate-specificantigen(PSA)andGleasonscore.Methods:Themaximum,meanandminimumelasticmodulusvalues(Emax,Emem,Eun)of62patientswithhistopathologicallyconfirmedmalignantprostatelesionsand59patientswithbenignlesionswereretrospectivelyanalyze.ROCcurvesweredrawntoanalyzethediagnosticefficacyofSWEcombinedwithelasticmodulusinprostatecancer,andtherelationshipsbetweenEmx,Emen,EmlevelsandPSA,Gleasonscorewereanalyzedinthemalignantgroup.Results:Emx,EmanandEmweresignificantlyhigherinthemalignantgroupthantho...
