现代泌尿外科杂志·基础研究·经尿道单孔腔镜膀胱肿瘤整块切除术的离体动物模型实验研究王伟峰1,张�军²,万建省",刘四明",邹(1.上海健康医学院附属周浦医院泌尿外科,上海201318;2.上海中医药大学研究生院,上海201203)Experimentalstudyofen-blocresectionofbladdertumorsbytransurethralsingle-portlaparoscopyinvivoanimalmodelsWANGWeifeng',ZHANGJun?,WANJiansheng',LIUSiming',ZOUYuan',ZHENGShaoqiu',HAOJidong',LIAOGuoqiang',GONGHua',OUYANGLeil(1.DepartmentofUrology,ShanghaiUniversityofMedicine&HealthSciencesAffiliatedZhoupuHospital,Shanghai201318;2.GraduateSchool,ShanghaiUniversityofTraditionalChineseMedicine,Shanghai201203,China)ABSTRACT:ObjectiveToexplorethefeasibilityofen-blocresectionofbladdertumorsbyflexiblecystoscopecombinedwithlaparoscopicinstrumentsthroughurethraandtoprovidereferencefortheclinicalapplicationofthistechnique.MethodsSelf-designedandprocessedtransurethralsingle-holePORTandOlympuselectroniccystoscopewereusedasobservationmiror;@l.8mmsoftgrasper,tissuescissors,electrichook,andultrasonicscalpelwereusedasinstruments;theporcinebladderwasusedasamodel.ThePORTwasplacedthroughtheurethra,andthecystoscopewasinsertedtoobservetheinnerwallofthebladderandtheconditionofthemucosa.Afterthelesionsitewasidentifiedinthebladdercavity,thesoftgrasperwasinsertedtopullthemucosatoberemoved,whichwasthenfixedwithtensionatthetargetpositiontomaintainasatisfactoryfeildofview.Thesurgeonheldthecystoscopeinthelefthand,andoperatedthelaparoscopicinstrumentsintothebladdercavitythroughthePORTwiththerighthand.Observingwiththecystoscopeandliftingandpullingthemucosawiththegrasper,thesurgeonsimulatedthecuttingandpushingactionstorealizetheen-blocresectionofthelesionedmucosa.ResultsThemucosaat4differentlocationsweresuccessfullyresectedon2invitroporcinebladdermodels.ConclusionTheinvitroexperimentsshowthatthecombinationofflexibleelectroniccystoscopeandlaparoscopicinstrumentsachievessynergisticeffectsinen-blocresectionofbladdertumorbytransurethralsingle-holelaparoscopewithoutadditionaliatrogenicbladderinjuryc...
