12论著影像研究与医学应用2024年1月第8卷第2期螺旋CT灌注成像技术在鉴别肝癌及肝硬化诊断中的应用价值张金彩1，隋汝德1，张妹2（通信作者）（1临沂市中心医院影像科山东临沂276400）（2泰安市立医院医学影像科山东泰安271000）TheapplicationvaluespiralCTperfusionimaginginthedifferentialdiagnosisoflivercancerandlivercirrhosisZHANGJincai1,SUIRude1,ZHANGMei2(Correspondingauthor)1DepartmentofMedicalImaging,LinyiCentralHospital,Linyi,Shandong276400,China;2DepartmentofMedicalImaging,Tai'anMunicipalHospital,Tai'an,Shandong271000,China【Abstract】ObjectiveToinvestigatetheapplicationvalueofspiralCTperfusionimaginginthedifferentialdiagnosisoflivercancerandcirrhosis.Methods89patientswithlivercancerandcirrhosisadmittedtoLinyiCentralHospitalfromAugust2021toAugust2023wereselected,allofwhomunderwentspiralCTperfusionimagingexamination.Pathologicalexaminationwasusedasthegoldstandard,andthedetectionanddiagnosticvalueofspiralCTperfusionimagingforlivercancerandcirrhosiswerestatisticallyanalyzed.ResultsAmongthe89patients,39casesoflivercancerand50casesofcirrhosisweredetectedpathologically;Amongwhich38casesoflivercancerand51casesofcirrhosisweredetectedbyspiralCTperfusionimaging,1casewasmisdiagnosedand2casesweremissed.ThesensitivityofspiralCTperfusionimagingwas94.87%(37/39),thespecificitywas98.00%(49/50),theaccuracywas96.63%(86/89),thepositivepredictivevaluewas97.37%(37/38)andthenegativepredictivevaluewas96.08%(49/51).KappatestshowedthattheKappavalueofspiralCTperfusionimagingindifferentialdiagnosiswas0.931,whichwashighlyconsistentwiththegoldstandard.Hepaticarteryperfusionvolume(HAP),hepaticbloodflow(BF),hepaticbloodvolume(BV)andcapillarysurfacepermeability(PS)inhepatocellularcarcinomagroupwerehigherthanthoseincirrhosisgroup,andthemeantransittime(MTT)wasshorterthanthatincirrhosisgroup,withstatisticalsignificance(P<0.05).ConclusionSpiralCTperfusionimaginghashighvalueinthedifferentialdiagnosisoflivercancerandcirrhosis,whichfacilitatesearlyidentificationofspecificdiseasesfortargetedtreatmentandbetterimprove...
