LETTERTOTHEEDITOROpenAccessExosomallongnoncodingRNAHOTTIPaspotentialnoveldiagnosticandprognosticbiomarkertestforgastriccancerRuiZhao1†,YanliZhang2†,XinZhang1,YongmeiYang1,XinZheng1,XiaohuiLi1,YingjieLiu1andYiZhang1*AbstractLongnoncodingRNAHOTTIPplaysimportantrolesinthegenerationandprogressionofhumancancers.Exosomesparticipateincellularcommunicationbytransmittingmolecularsbetweencellsandareregardedassuitablecandidatesfornon-invasivediagnosis.However,theexistenceofHOTTIPinthecirculatingexosomesandthepotentialrolesofexosomalHOTTIPingastriccancer(GC)waspoorlyunderstood.ThisstudyaimsatinvestigatingtheclinicalrolesofexosomalHOTTIPinGC.SerumexosomalHOTTIPfrom246subjects(126GCpatientsand120healthypeople)weredetectedbyreversetranscriptionreal-timequantitativepolymerasechainreaction(RT-qPCR).OurresultsshowedthatexpressionlevelsofexosomalHOTTIPweretypicallyupregulatedinGCthaninnormalcontrol(P<0.001).Anditsexpressionlevelsweresignificantlycorrelatedwithinvasiondepth(P=0.0298)andTNMstage(P<0.001).TheAUCforexosomalHOTTIPwas0.827,whichdemonstratedahigherdiagnosticcapabilitythanCEA,CA19–9andCA72–4(AUC=0.653,0.685and0.639,respectively)(P<0.001).TheKaplan–MeieranalysisshowedacorrelationbetweenincreasedexosomalHOTTIPlevelsandpooroverallsurvival(OS)(logrankP<0.001).AndunivariateandmultivariateCOXanalysisrevealedexosomalHOTTIPoverexpressionwasanindependentprognosticfactorinGCpatients(P=0.027).ThesefindingsdemonstratedthatexosomalHOTTIPmaybeapotentialbiomarkerforGCindiagnosisandprognosis.Keywords:Gastriccancer,LongnoncodingRNA,HOTTIP,Diagnosis,PrognosisGastriccancer(GC)isahugeburdenworldwidewithhighmorbidityandhighmortality,especiallyinEasternAsia[1].Despitemanyadvancesinthediagnosisandtreatment,theprognosisofGCpatientsremainspoor,withover70%patientsultimatelydiedfromthisdisease[2].EarlydiagnosisandearlytreatmentisconducivetoimprovingprognosisofGC.ThegoldstandardforGC,endoscopyfollowedbypathologicalexamination,isinva-siveanduncomfortable[3].Bodyfluids-basedtestingisconsideredtobe...
