临床医学研究与实践2023年3月第8卷第9期Effectoflow-dosedopaminecombinedwithtolvaptanoncardiacfunctionrecoveryinpatientswithrefractoryheartfailureMAOChunfa(TongzhaipuTownHealthCenterofTangheCounty,Nanyang473404,China)ABSTRACT:ObjectiveToinvestigatetheeffectoflow-dosedopaminecombinedwithtolvaptanonthecardiacfunctionrecoveryinpatientswithrefractoryheartfailure.MethodsAtotalof68patientswithrefractoryheartfailureadmittedinourhospitalfromMarch2021toMarch2022wereselectedastheresearchobjects.Thepatientswererandomlydividedintoconventionalgroup(34cases,tolvaptantreatment)andobservationgroup(34cases,low-dosedopaminecombinedwithtolvaptantreatment).Theclinicalefficacy,cardiacfunctionindexes,myocardialmarkers,inflammatoryfactorslevelsandadversereactionswerecomparedbetweenthetwogroups.ResultsThetotaleffectiverateoftreatmentintheobservationgroupwassignificantlyhigherthanthatintheconventionalgroup,andthedifferencewasstatisticallysignificant(P<0.05).Beforetreatment,therewerenosignificantdifferencesinleftventricularejectionfraction(LVEF),leftventricularendsystolicdiameter(LVESD),leftventricularendsystolicvolume(LVESV)andleftventricularenddiastolicvolume(LVEDV)betweenthetwogroups(P>0.05);aftertreatment,theLVEFintheobservationgroupwassignificantlyhigherthanthatintheconventionalgroup,theLVESD,LVESVandLVEDVweresignificantlylowerthanthoseintheconventionalgroup,andthedifferenceswerestatisticallysignificant(P<0.05).Beforetreatment,therewerenosignificantdifferencesinthelevelsofB-typenatriureticpeptide(BNP),cardiactroponinT(cTnT),solublegrowthstimulationexpressedgene2protein(sST2)andlacticacid(LA)betweenthetwogroups(P>0.05);aftertreatment,thelevelsofBNP,cTnT,sST2andLAintheobservationgroupweresignificantlylowerthanthoseintheconventionalgroup,andthedifferenceswerestatisticallysignificant(P<0.05).Beforetreatment,therewerenosignificantdifferencesinthelevelsoftumornecrosisfactor-α(TNF-α),C-reactiveprotein(CRP),interleukin-6(IL-6)andintercellularadhesionmolecule-1(ICAM-1)betweenthetwogroups(P>0.0...
