1002022Vol.9No.11ElectronicJournalofPracticalGynecologicalEndocrinology•诊断技术•HPV感染检测在基层妇女宫颈病变筛查中的应用分析秦秀朵▲，司园园（东部战区总医院淮安医疗区妇产科，江苏淮安223001）【摘要】目的分析将人乳头瘤状病毒（HPV）感染检测应用于基层妇女宫颈病变筛查中的效果。方法选取于本院接受宫颈病变筛查的妇女100例为研究对象，对其进行薄层液基细胞学（TCT）与高危型HPV-DNA检测，检测结果阳性者接受阴道镜下多点宫颈组织活检，以此评价两种筛查方法单一使用及联合使用的应用价值并分析HPV感染情况。结果联合检测的敏感度、特异度及阴性预测值均明显优于单一检测，差异有统计学意义（P＜0.05）；HPV-DNA检测的敏感度、阳性预测值及阴性预测值均略高于TCT检测，但二者比较，差异无统计学意义（P＞0.05）。HPV-DNA阳性患者中，30岁以上妇女的感染率为68.29%，明显高于30岁以下的31.71%，差异有统计学意义（P＜0.05）。结论HPV-DNA阳性高发于宫颈癌前病变妇女，联合HPV与TCT进一步检测宫颈病变性质，其筛查意义与检测价值对30岁以上妇女更加显著。【关键词】HPV感染检测；基层妇女；宫颈病变筛查ApplicationanalysisofHPVinfectiondetectionincervicallesionsscreeningingrass-rootswomenQinXiuduo,SiYuanyuan(DepartmentofObstetricsandGynecology,HuaianHospitaloftheEasternTheaterGeneralHospital,Huaian223001,China)【Abstract】ObjectiveToanalyzetheeffectofhumanpapillomavirus(HPV)infectiondetectionintheofcervicallesionsscreeningingrass-rootswomen.MethodsAtotalof100womenwhowerescreenedforcervicallesionsinourhospitalwereselectedastheresearchobjects.Thinprepcytologictest(TCT)andhigh-riskHPV-DNAweretestedforthem.Thosewhotestedpositiveunderwentcolposcopyformultiplecervicalbiopsies.TheapplicationvalueofsingleorcombineduseofthetwoscreeningmethodswasevaluateandandthestatusofHPVinfectionwasanalyzed.ResultsThesensitivity,specificityandnegativepredictivevalueofcombineddetectionweresignificantlybetterthanthoseofsingledetection(P＜0.05);thesensitivity,positivepredictivevalueandnegativepredictivevalueofHPV-DNAtestwereslightlyhigherthanthoseofTCTtest,buttherewerenosignificantdifferencesbetweenthem(P＞0.05).AmongHPV-DNApositivepatients,theinfectionrateo...
