58·中国CT和MRI杂志2023年02月第21卷第02期总第160期【第一作者】张庆团，男，副主任医师，主要研究方向：呼吸结核。E-mail：Zhangqingtuan@sina.com【通讯作者】张庆团论著TheApplicationValueofLow-doseCTCombinedwithSerumTumorMarkersintheDiagnosisofEarlyNon-smallCellLungCancerZHANGQing-tuan1,*,GUOXing-quan2,WANGJia-ci3.1.DepartmentofInternalMedicine,LongtanHospital,GuangxiZhuangAutonomousRegion,Longtan545005,GuangxiProvince,China2.DepartmentofImaging,LongtanHospital,GuangxiZhuangAutonomousRegion,Longtan545005,GuangxiProvince,China3.GeneralDepartment,ZhongshanHospitalofTraditionalChineseMedicine,Zhongshan52840,GuangdongProvince,ChinaABSTRACTObjectiveToinvestigatetheclinicalvalueoflow-doseCT(LDCT)combinedwithserumtumormarkersintheearlydiagnosisofnon-smallcelllungcancer(NSCLC).MethodsThepatientswithsuspectedearlyNSCLC(167cases)whowerescreenedinGuangxiZhuangAutonomousRegionLongtanHospitalfromFebruary2016toMarch2020wereselectedastheresearchobjects,andtheclinicaldataofpatientswerecollected.Pathologicaldiagnosiswasthe“goldstandard”,andtherewere137casesearlyNSCLCand30casesbenignlesions.ResultsEarlyNSCLChasavarietyofimagingfeatures,mainlysolidlesions,followedbypartialsolidnodules,rarenon-solidnodularlesions.Comparedwithpatientswithbenignlesions,patientswithearlyNSCLChadhigherserumcarcinoembryonicantigen(CEA),cytokeratinfragment21-1(CYFRA21-1)andneuronspecificenolase(NSE)levels(P<0.05).TheKappacoefficientindiagnosisofLDCTcombinedwithserumtumormarkerswashigherthanthatindiagnosisofLDCTandtumormarkers,andtheconsistencywithpathologicaldiagnosiswasthestrongest(P<0.05).TheaccuracyandsensitivityofLDCTcombinedwithserumtumormarkerswerehigherthanthatofLDCTandtumormarkers,andthedifferencewasstatisticallysignificant(P<0.05).ConclusionLDCTcombinedwithserumtumormarkerscansignificantlyimprovethesensitivityandaccuracyofearlyNSCLCdiagnosisandtheconsistencywithpathologicaldiagnosisresults,effectivelyreducemisdiagnosisandmisseddiagnosis,whichisworthyofclinicalpromotion.Keywords:Low-doseCT;SerumTumorM...
